While members of WVHPCN manage the palliative care provision of services for the patients we serve, effective palliative care requires a collaborative effort from a diverse team of healthcare providers, known as the patients’ “interdisciplinary team.”

 

Interdisciplinary collaboration lies at the heart of effective palliative care, where physicians, nurses, social workers, chaplains, and other professionals work together to address the physical, emotional, and spiritual needs of patients. Palliative care is only able to provide holistic and compassionate support to both patients and their families by harnessing the collective expertise and perspectives of various team members.
